Spain Digital Nomad Visa Health Insurance Requirements
Inadequate health insurance is one of the top three reasons the UGE-CE rejects Digital Nomad Visa applications in 2026. The requirement sounds straightforward — you need private health insurance — but the specific criteria disqualify most international policies, travel insurance plans, and even some Spanish policies that are marketed to expats but do not meet the visa standard. Buying the wrong policy means starting your application over.
What the Law Requires
Spain's Digital Nomad Visa regulation requires private health insurance that provides coverage equivalent to the Spanish National Health System (Sistema Nacional de Salud). In practice, the UGE-CE looks for four specific characteristics:
The insurer must be authorized to operate in Spain. International insurers based in the US, UK, or elsewhere that are not registered with the Spanish insurance regulator (Dirección General de Seguros) are not acceptable, even if they provide worldwide coverage. The policy must come from a company licensed in Spain.
No copayments (sin copagos). Policies with any form of copayment — even small fees per consultation — do not meet the requirement. This is the most common disqualifying feature. Many "basic" Spanish health plans charge €5–€15 per GP visit or specialist appointment. These are rejected.
No waiting periods (sin carencias). A carencia is a waiting period before certain treatments or services become available. Many standard health insurance policies in Spain have waiting periods for surgeries, specialist consultations, or maternity care. Policies with any carencias are rejected.
Full territorial coverage in Spain. Care-only travel insurance or policies that cover Spain only for emergencies and repatriate you elsewhere for treatment are not sufficient. You need full comprehensive coverage within Spanish territory.
Insurers That Generally Meet the Requirement
Several Spanish insurers offer policies that are structured to meet the Digital Nomad Visa requirements. The most commonly used are:
Sanitas. One of the largest private health insurers in Spain and the most frequently cited by immigration lawyers as reliably visa-compliant. Their "Digital Nomad Visa" oriented plans are designed with the carencia and copago requirements in mind, though you should verify the specific policy terms before purchasing.
Adeslas. Another major insurer with plans that can be configured to meet visa requirements. Their "Adeslas Total" product line is commonly used by DNV applicants.
Asisa. A mutual insurer with a strong hospital network across Spain, particularly useful if you intend to live outside Madrid or Barcelona.
DKV. Less commonly discussed but used by some applicants, particularly in the Basque Country and Catalonia where their network is strong.
When obtaining a policy, explicitly tell the insurer you need coverage for a Digital Nomad Visa application and ask them to confirm in writing that the policy has no copayments and no waiting periods. Request a policy summary (carta de condiciones) that explicitly states "sin copagos" and "sin carencias" — you will include this document in your application file.
What Does Not Qualify
Travel insurance. No travel insurance policy, regardless of how comprehensive it appears, meets the requirement. Travel insurance is designed for emergencies and short trips, not equivalent-to-SNS coverage. Do not submit one.
International expat health insurance. Policies from Cigna Global, Allianz Care, AXA International, or similar internationally positioned insurers may provide excellent coverage but are typically not authorized by the Spanish insurance regulator in the way required for the visa. Check authorization status carefully before purchasing.
Policies with any copago. If you already have a Spanish health insurance policy that you obtained independently before starting the visa process, check whether it has copayments. Many Spanish residents hold plans with €5–€20 GP consultation fees — comfortable to live with, but disqualifying for the visa.
NHS entitlement or EHIC. If you are a UK national who is still entitled to NHS coverage, or if you have an EHIC for travel within Europe, neither of these counts. The requirement is for a private policy from an authorized Spanish insurer.

Cost
Visa-compliant health insurance in Spain typically costs €600–€1,500 per year for a healthy adult under 45. Premiums are age-banded and increase meaningfully after 50 and again after 60. Expect to pay the full annual premium upfront or in a few installments — monthly payment plans are available from some insurers but not all.
For family applications, each family member included in the DNV application must have their own qualifying policy. You cannot use one policy to cover multiple adults. Children's policies are generally less expensive — €200–€500 annually depending on age and insurer.
Timing the Purchase
Buy your health insurance before finalizing your application file. The policy must be active at the time of submission, and you will include the policy certificate and payment confirmation as supporting documents.
For the consular path (applying from outside Spain), some consulates accept policies that start on the date of entry into Spain rather than the date of application. Check with your specific consulate — requirements vary. For the UGE-CE in-country path, the policy must be active at the date of application.
Also note: your health insurance requirement does not end when your TIE is issued. It must remain active throughout your entire residency period. When you apply for renewal, you will need to demonstrate continued coverage. Let the policy lapse and you risk complications at renewal.
