UAE Visa Medical Test: What's Tested, What Fails, and What to Expect
The medical fitness test is the part of the UAE employment visa process that causes the most anxiety — and the most avoidable confusion. Most concerns people have are based on outdated information. The UAE's approach to several conditions, particularly tuberculosis, has changed significantly in recent years.
Here is exactly what the test involves and what the different outcomes mean.
What the Medical Test Screens For
The UAE residence medical fitness test is mandatory for all applicants aged 18 and above. It consists of two components:
Blood test: Screens for HIV/AIDS, syphilis (active infection), and Hepatitis B or C (for specific professional categories — see below).
Chest X-ray: Screens for active pulmonary tuberculosis (TB).
The test is conducted at government-approved clinics. In Dubai, the primary centres are Smart Salem (operated by the Dubai Health Authority), with multiple locations across the emirate. In Abu Dhabi, SEHA-approved centres conduct the test. Results from non-approved clinics are not accepted by MOHRE or ICP.
What Results Cause Visa Rejection
| Condition | Outcome |
|---|---|
| Active HIV/AIDS | Visa denied; no retest option; subject to deportation |
| Active pulmonary tuberculosis | Visa denied; deportation |
| Active syphilis | Conditional — requires treatment, after which a retest is permitted |
| Leprosy | Visa denied |
HIV is an absolute bar. There is no appeal, exception, or conditional path. This is one of the few non-negotiable restrictions in the UAE medical system.
The Inactive TB / Old Lung Scar Rule — What Changed
This is where many applicants from high-TB regions (India, Pakistan, Philippines, Bangladesh, Indonesia, parts of Africa) have unnecessary anxiety based on outdated information.
Previous rule: Any evidence of old TB on an X-ray — including healed, inactive scars from a childhood infection — could result in visa denial or deportation.
Current rule (2026): Applicants with inactive TB (old scars, calcified nodules, or non-infectious findings) are no longer automatically rejected. Instead, they receive a "Conditional Fitness" certificate and are issued a one-year residency visa. They must attend periodic health monitoring at designated government health centres during their stay.
This change matters enormously for applicants from high-TB-prevalence countries. An old chest X-ray that shows healed TB from 20 years ago is no longer an automatic disqualification — it results in a conditional certificate and normal residency, just with annual monitoring.

Hepatitis B: When It Is and Is Not Tested
Standard employment visa medical tests screen for HIV, syphilis, and TB. Hepatitis B and C are additionally tested for applicants in specific professional categories:
- Healthcare workers (doctors, nurses, dental professionals, lab technicians)
- Teachers and nursery workers
- Domestic workers
- Food handlers
Since 2026, healthcare, education, and domestic workers must also show proof of Hepatitis B vaccination. If you have no vaccination record, you must receive the three-dose series in the UAE — your visa remains conditional until the course is complete.
For office-based professionals, IT workers, finance, and most other roles, Hepatitis B is not part of the standard screen.
How to Prepare for the Medical Test
Book in advance: Smart Salem centres in Dubai can be booked through the DHA Smart App or website. Walk-ins are accepted but queues at main centres (especially Bur Dubai) can be long. Centres in Al Quoz and Deira typically have shorter waits.
What to bring:
- Original passport
- Emirates ID (or a copy if not yet issued)
- Employer's work permit reference number (your employer or PRO will provide this)
- Two passport photographs (some centres photograph you on-site)
- Payment (approximately AED 300–700 depending on the centre and tests required)
Day before: Avoid taking supplements or medications with biotin, which can affect some blood test results. There are no fasting requirements for this test.
Results: Standard results take 24–48 hours. Smart Salem's express service in Dubai delivers results in 30 minutes. Results are sent digitally to the MOHRE/ICP system — you do not need to physically collect a paper certificate in most cases.
What Happens If You Fail
For HIV, syphilis (active), or leprosy: the visa application is rejected and you will receive a letter advising you to make arrangements to leave. There is no appeal mechanism.
For active TB: the same outcome applies — the visa is denied, but treatment options exist in your home country.
For inactive TB (conditional fitness): you are cleared, issued the conditional certificate, and your employer proceeds with the residency process. The monitoring schedule is organised by the health authority, not your employer.
Does the Medical Test Apply to Renewals?
Yes. The medical fitness test is required at each renewal of an employment visa, not just at initial issuance. The same tests are conducted. Centres maintain records from previous tests, so long-standing residents undergo the same process as new arrivals.
The UAE Employment Visa Guide includes a preparation checklist for the medical test and explains the conditional TB fitness path in detail — along with how to handle the scenario where your employer tries to make you pay the clinic fees yourself.
