189 Visa Document Checklist: What to Prepare Before Your Invitation

The 60-day window after receiving an Invitation to Apply is fixed by legislation. No extensions. This is the point where applicants who prepared in advance move smoothly and those who start gathering documents from scratch face a scramble.

Every document in this checklist needs to be collected before your invitation arrives — not after. Most of these items have lead times of weeks, not days.

Why You Cannot Wait for the Invitation

When the Department of Home Affairs issues an Invitation to Apply, you have 60 calendar days to:

  • Lodge a complete visa application via ImmiAccount
  • Upload certified copies of all supporting documents
  • Pay the Visa Application Charge
  • Initiate health examinations and police clearances

Police clearances from some countries take 4 to 8 weeks. Health examination bookings at Bupa Medical Visa Services can take 1 to 3 weeks for appointments. Document certification and translation (if required) adds further time. On a 60-day clock, starting from zero is a serious risk.

Identity Documents

Every applicant (primary and all dependents) needs:

  • Current passport: bio-data pages (photo, personal details, validity dates)
  • All previously held passports that cover any period being claimed (employment, study)
  • National identity card (if applicable)
  • Name change certificate (if your name differs from that on any other document — marriage certificate, deed poll, etc.)

All identity documents must be certified. Offshore applicants use a notary public or equivalent official. Onshore applicants can use a Justice of the Peace, registered migration agent, or lawyer.

Employment Evidence

This is the most heavily scrutinized category. The Department of Home Affairs verifies skilled employment both against your EOI claims and against your skills assessment outcome. The standard of evidence required is high.

Reference letters must include:

  • Official company letterhead with full address, phone number, email, and website
  • Exact start and end dates in DD/MM/YYYY format ("three years" or "since 2021" is rejected)
  • Whether the position was full-time or part-time, permanent or contract
  • Exact hours worked per week
  • At least five specific duty descriptions using active language that mirrors the ANZSCO task list for your nominated occupation — not generic summaries
  • Base salary and significant allowances
  • Signature of direct supervisor or authorized HR representative, with their name, title, and direct contact details

Reference letters alone are not sufficient. They must be triangulated with:

  • Payslips covering the full period claimed
  • Tax records (Australian ATO Notices of Assessment for onshore periods; overseas tax documents for offshore periods)
  • Superannuation / provident fund statements where applicable

For any period where employment is claimed but the employer no longer exists or is uncontactable, a Statutory Declaration from a senior colleague who can verify the role and duties may be accepted — but this is a weaker form of evidence and should be supplemented with whatever documentation is available.

Qualifications

  • Degree certificate(s) or diploma(s) — original or certified copy
  • Academic transcripts covering all years of study
  • Skills assessment result letter from the relevant authority (ACS, EA, VETASSESS, ANMAC, etc.)

If your qualifications are in a language other than English, they must be officially translated. In Australia, translation must be by a NAATI-accredited translator. Offshore, an equivalent official translator with a certification stamp and credentials is accepted.

English Language Test Results

  • Test result certificate (IELTS, PTE Academic, TOEFL iBT, OET, or Cambridge C1 Advanced)
  • Results are valid for 3 years from the test date for visa purposes

Ensure the test date is within the 3-year window at the time of your visa application, not just at the time of your EOI submission. If your test expires between EOI submission and visa application, you will need to retest.

Health Examination

Health examinations must be conducted by an approved medical practitioner — Bupa Medical Visa Services for applicants in Australia, or an approved panel clinic for offshore applicants. The Department does not accept results from your personal GP.

The examination includes a general health assessment, chest X-ray (for adults), and blood tests. Book early — Bupa's schedule in major cities can be several weeks out.

If you have a pre-existing condition, the case officer applies the Significant Cost Threshold (currently $86,000 AUD over a 5-year projection) to determine whether the health requirement is met. The Subclass 189 has no health waiver provision — a failed health assessment results in mandatory refusal. If you have a family member with a significant health condition (including children), seek advice before lodging.

Police Clearances

You need a national police clearance (or equivalent) from every country where you spent 12 or more cumulative months in the past 10 years. This includes Australia if you have lived here.

  • Australia: National Police Check via ACIC — approximately $42 AUD, typically 1 to 2 weeks for online applications
  • India: from the Passport Office or through the Embassy
  • UK: from the Disclosure and Barring Service (DBS)
  • US: FBI Identity History Summary
  • Other countries: from their national or relevant state/provincial police authority

Clearances must be translated if not in English. Some countries take 6 to 8 weeks — request them as early as possible.

Form 80: Personal Particulars

Form 80 (Personal Particulars for Character Assessment) is a long biographical form that every primary applicant must complete. It asks about:

  • All addresses you have lived at (complete address, dates, with no gaps)
  • All employment history (employer, address, dates, duties)
  • All education (institution, dates, qualification)
  • All passports ever held
  • Military service
  • Any involvement in criminal proceedings, court actions, or legal judgments
  • Visa refusals or cancellations from any country
  • Political or paramilitary affiliations

Form 80 is one of the highest-risk documents in the application. Omissions or inaccuracies — even unintentional ones — can trigger a Public Interest Criterion 4020 assessment (character/fraud concerns) which may result in refusal and a visa ban.

Take the time to complete Form 80 carefully and accurately. Cross-reference it against your passport, employment records, and tax history to ensure no gaps in the timeline. If you lived somewhere for 11 months but rounded up to 12 on the form, that discrepancy can become a problem.

Relationship Evidence (If Claiming Partner Points or Including Dependents)

If you are including a partner on the visa or claiming partner points:

  • Marriage certificate or evidence of de facto relationship (varies by state and country of relationship)
  • Relationship evidence spanning at least 12 months: joint lease/mortgage, shared bank statements, photos together, correspondence, communication records, travel records
  • For long-distance relationships: international transfer records, chat logs, travel itineraries, statutory declarations from friends and family

Supplementary Point Evidence

For any supplementary points claimed:

  • NAATI CCL: certificate of credential from NAATI
  • Professional Year: completion certificate from the registered provider
  • Australian Study Requirement: transcripts and attendance records from the Australian institution
  • Regional Study: university enrollment records showing campus address
