Both routes get you a UK work visa. But if you are a healthcare professional, treating them as equivalent is a mistake that could cost your family over £22,000 in unnecessary fees — and potentially price you out of eligibility entirely.

The Health and Care Worker visa is a sub-category of the Skilled Worker route, but the differences are substantial. Here is a direct comparison across every dimension that matters for an international nurse, doctor, or allied health professional.

Who Can Use Each Route

Skilled Worker visa: Any job on the eligible occupation list at RQF Level 6 (degree equivalent) or above. Since July 2025, the skill floor was raised from RQF Level 3 to RQF Level 6. Engineers, accountants, IT specialists, and a wide range of degree-level professionals use this route.

Health and Care Worker visa: Only specific healthcare roles with specific Standard Occupational Classification (SOC) codes. Eligible roles include registered nurses (SOC 2231), medical practitioners (SOC 2211), pharmacists (SOC 2213), paramedics (SOC 3213), physiotherapists (SOC 2221), occupational therapists (SOC 2222), and some others. Crucially, your employer must be an NHS body, NHS supplier, or Care Quality Commission (CQC)-registered adult social care provider. A private hospital that is not a CQC-registered entity cannot use this route.

If your job and employer qualify for the Health and Care Worker visa, you should always use that route — it is strictly more advantageous in every financial dimension.

Applying for a 5-year visa as a single applicant? £628 on the H&CW route versus £1,636 on the standard Skilled Worker route. Add a spouse and two children and that becomes £2,512 versus £6,544 — before you even get to the IHS.

Immigration Health Surcharge

This is where the financial difference becomes dramatic.

Standard Skilled Worker: £1,035 per adult per year, £776 per child per year. For a 5-year visa covering a couple with two children: £10,350 + £10,350 + £7,760 = £18,460.

Health and Care Worker: £0 — completely exempt, for the main applicant and all eligible dependents.

Total upfront visa cost comparison for a family of four on a 5-year visa:

  • Standard Skilled Worker: approximately £24,582
  • Health and Care Worker: approximately £2,512

That £22,000 difference is not a rounding error. For a nurse from Zimbabwe or Nigeria, where monthly nursing salaries run to a few hundred US dollars, this is literally the difference between being able to finance the move or not.

The exemption exists because healthcare workers are being brought in to staff the NHS — charging them to fund the NHS they are about to work in was recognized as circular and counterproductive.

Salary Thresholds

Standard Skilled Worker: The general minimum salary threshold (as of post-July 2025 changes) is £41,700, or the going rate for the specific occupation — whichever is higher. For most healthcare occupations, the occupation going rate is the relevant figure, and it varies widely.

Health and Care Worker: Healthcare professionals employed on NHS Agenda for Change (AfC) pay scales are exempt from the £41,700 threshold. They must simply be paid at or above their AfC banding rate, with a minimum floor of £25,000. A Band 5 nurse starting at £31,049 on AfC comfortably meets this — and does not need to worry about the £41,700 test.

For roles not on a national pay scale (some private sector healthcare positions), the ISL discount applies: the minimum is £25,000 or 100% of the occupation going rate, whichever is higher.

Salary Thresholds for ILR (Settlement)

After 5 years, you apply for Indefinite Leave to Remain. The ILR salary rules also differ:

Standard Skilled Worker ILR: £41,700 or the occupation going rate.

Health and Care Worker ILR (AfC): Simply your current AfC banding rate — no £41,700 test for NHS clinicians.

Nurses, doctors, and allied health professionals on NHS contracts do not hit a settlement salary cliff. At Band 5 or above, they automatically qualify at the point of their ILR application.

Grandfathering note: Workers who obtained their first visa before 4 April 2024 can apply for ILR at the lower threshold of £31,300, provided they apply before December 2026.

Supplementary Employment

This is one area where the routes diverge in a nuanced way that many visa holders do not know about.

Standard Skilled Worker (pre-July 2025 rule): Could work up to 20 additional hours per week in almost any listed shortage occupation.

Both routes (post-July 2025): Supplementary employment is restricted. Workers can only work supplementary hours in the same occupation code as their primary sponsored role, or in roles at RQF Level 6 or above. An NHS nurse (SOC 2231, RQF Level 6) can still pick up care home shifts as a supplementary job because their primary role is at Level 6. A care worker (SOC 6135, RQF Level 3) cannot freely work in other sectors.

Workers who held continuous Skilled Worker visas before 22 July 2025 retain the older, broader supplementary employment rights.

Eligible Employer Requirement

Standard Skilled Worker: Any Home Office approved sponsor, in any sector.

Health and Care Worker: Restricted to NHS bodies, NHS suppliers, and CQC-registered adult social care providers. If a private hospital, clinic, or care home does not appear on the CQC register, they cannot issue Health and Care Worker visa CoS documents — they would have to use the standard Skilled Worker route, which means you lose the fee and IHS advantages.

Always check the CQC register for private employers before assuming the H&CW route is available. The care home or private clinic needs an active, valid CQC registration in your target care category.

Processing Time

Standard Skilled Worker: Standard processing up to 8 weeks.

Health and Care Worker: Typically within 3 weeks of biometrics — expedited due to the NHS recruitment priority.

Route to Settlement

Both routes lead to Indefinite Leave to Remain after 5 years of continuous residence. The rules and eligibility tests are effectively the same, with the AfC salary exemption noted above being the main practical difference at the ILR stage.

Which Route Should You Use?

If your job and employer qualify for the Health and Care Worker visa, always use it. There is no scenario where the standard Skilled Worker route is financially superior for an eligible healthcare professional.

The only time you end up on the standard Skilled Worker route is if:

  • Your specific occupation code is not listed for the H&CW visa
  • Your employer is a private healthcare entity not registered with the CQC
  • Your role falls below degree level and is not on the Health and Care ISL exception list
